According to the report, Dubbs was driving southbound on County Road 575 in his 1997 Mazda b2300 pickup truck when for an unknown reason, he failed to negotiate a curve causing the truck to leave the roadway.

The vehicle crashed through some traffic signs and overturned ejecting Dubbs out of the vehicle.

According to the report, Dubbs was not wearing his seatbelt at the time of the incident.
